What is infertility, and what causes it?

Technically defined as “not being able to get pregnant after one year of trying (or six months if a woman is 35 or older),” infertility can lie with either partner. Infertility can stem from many factors, ranging from physiological issues to lifestyle factors. 

Among the primary contributors to infertility are:

  • Ovulatory disorders: Irregular ovulation or the absence of ovulation altogether can significantly impair fertility. Conditions such as polycystic ovary syndrome (PCOS) and premature ovarian failure (POF) are common culprits.
  • Sperm disorders: Male infertility can be attributed to factors such as low sperm count, poor sperm motility, or abnormal sperm morphology. These issues can hinder the fertilization process, impacting the chances of conception.
  • Tubal factors: Structural abnormalities or blockages in the fallopian tubes can obstruct the passage of sperm or eggs, preventing fertilization from occurring. Previous pelvic inflammatory disease (PID) or endometriosis can contribute to tubal damage.
  • Thyroid disorders: Thyroid dysfunction, whether hypothyroidism or hyperthyroidism, can disrupt hormonal balance and interfere with ovulation and menstrual regularity.
  • Pituitary disorders: Conditions affecting the pituitary gland, such as hyperprolactinemia, can disrupt the secretion of reproductive hormones, impairing fertility.
  • Age: Advanced maternal age is a significant factor in female infertility, as ovarian reserve declines with age. Similarly, age-related changes in sperm quality can impact male fertility.
  • Smoking and substance abuse: Tobacco use, excessive alcohol consumption, and recreational drug use can all negatively impact fertility in both men and women.
  • Obesity: Obesity is associated with hormonal imbalances and ovulatory dysfunction in women, while in men, it can lead to decreased sperm quality and erectile dysfunction.
  • Genetic disorders: Inherited genetic conditions, such as chromosomal abnormalities or single-gene mutations, can impact fertility in both men and women.

While infertility can be a daunting challenge, it's important to remember that help and support are available. By consulting with fertility specialists, undergoing comprehensive evaluations, and exploring treatment options such as assisted reproductive technologies (ART), individuals and couples can take proactive steps towards achieving their dream of parenthood.


Understanding the emotional terrain

The emotional toll of infertility is profound, impacting individuals and couples on multiple levels. On top of all the emotional strain of infertility, additional stress can be felt from the financial implications. Specialists, operations, IVF (in vitro fertilisation) and other treatments — all these can be effective, but they can also be incredibly expensive. These expenses often add to the stress and conflict often felt by couples experiencing infertility, as they question their motivations and ability to shoulder the additional burden.

The silent struggle of secondary infertility 

While primary infertility is more frequently discussed openly these days, secondary infertility remains a silent struggle for many. This lesser-known phenomenon refers to the inability to conceive or carry a pregnancy to term after previously giving birth to a child. The complexities of secondary infertility can be particularly challenging, as individuals navigate feelings of guilt, inadequacy, and confusion amidst the joys of parenthood. 

Secondary infertility can be caused by many of the same issues that cause primary infertility, with the added layer of confusion over the difficulty conceiving after having no problems doing so the first time. Couples are usually older, parenting an existing child or children, and are typically working at jobs and careers that have progressed since their first baby. All these factors — and so many others — can combine to cause secondary infertility.

Amidst the challenges of infertility, it's essential to nurture hope and resilience. Finding moments of joy, practising self-care, and maintaining open communication with partners can help alleviate the emotional burden of fertility struggles. Additionally, exploring alternative paths to parenthood, such as adoption or surrogacy, can offer new avenues for building a family.

National Infertility Awareness Week serves as a reminder of the importance of building a supportive community for individuals and couples facing fertility challenges.
